I am Jamshed, one of your previous patients, and I visited your practice with Roger. I moved back to Tajikistan and am reaching out to you for some crucial advice.

Unfortunately, today my 11-year-old son had an accident and broke his two upper front teeth (please see the attached photo). Given the challenges in finding specialized dental care here in Tajikistan (You are aware of it from my history), I immediately thought of you, the most skilled and knowledgeable dentist I know.

Could you kindly review the attached photo and provide your professional recommendation on the best method to restore my son's teeth? Your guidance would be immensely appreciated.

I am particularly concerned about seeking dental treatment here due to previous experiences where local dentists caused more harm, making future restoration difficult. Your expertise and advice would provide us with much-needed reassurance. Thank you very much for your time and assistance."

"Certainly, I remember you. I understand your worry and concern. This is traumatic for everyone involved. However, as much as I would love to be of service to you and your child, the ability to make an accurate diagnosis from one photograph is impossible. The patient really needs to be seen in person. If you were in America, I most certainly could help. There are many scenarios that could be at play here and more information, like clinical exams, x-rays, and CBCTs are required.

Root canals, pulpectomy, stabilization of teeth, if loose. in conjunction with occlusion adjustments and restorations are just a few things the dentist needs to consider. CONSERVATIVE as possible is always the best way to handle something like this. I wish I could be of more help."

"Thank you very much for your email and support. I truly appreciate it.

We visited three dentists here. Two of them recommended a root canal for one tooth and a filling for the second without a root canal. The third dentist suggested root canals for both teeth.

We decided to go with the first option and have an appointment scheduled for tomorrow. The dentist will perform a root canal on the larger broken tooth and attempt a filling for the second one.

Currently, my son is not in any pain, but his big broken tooth is sensitive to cold and warm temperatures.

I have attached an X-ray for your reference. Thank you again for your guidance."

"Your selection looks solid. For consideration, In the absence of visible radiographic or clinical swelling or pain, in lieu of a root canal, there are materials available to protect the pulp(nerve), even if there is a small (?) exposure of the nerve so no root canal is required. BioDentine is one I use. Look it up. It would not hurt to ask your dentist if he/she is aware of these treatment options. "

"Thank you sooo much! Today after showing your email to the dentist, he decided to save both, with no root canal. He does not have BioDentine, he found Proroot MTA. Today he applied it. Tomorrow we have another appointment. You are the man! I am so happy, thank you very much!!!!!!

Have a great weekend!"

"Excellent. Proroot MTA is not Bio Dentine but is a comparable product. You’re welcome. That will be 300.00 USD."

"I hope this message finds you well. I wanted to take a moment to express my sincere gratitude for your invaluable assistance. And yes, your sense of humor certainly brought a smile to my face!

I also wanted to share some positive news. The clinic we visited, "Eurodent," is considered the top dental clinic in Tajikistan. Several dentists there were already familiar with your suggestions, and I truly believe that your insights will continue to benefit many others who might face similar situations. It’s remarkable to see the impact your expertise can have even from afar.

Attached, you'll find the photo of the result. I look forward to the possibility of seeing you again, whether that’s here in Tajikistan or back in the U.S.

Wishing you a wonderful weekend."
